I picked up one of my hens today and noticed her nostril is completely clogged on one side. Is there something I can do for this? It seems incredibly hard.

That looks like food plus mucus secretions clogging the nostril. Try a few drops of hydrogen peroxide on QTips to soak the nostril, and leave it for a few minutes to soften the plug. Then take a wooden toothpick, and gently try to dig out the plug. Note that it may take several sessions.

Thank you!! Should I add anything to it after I am able to get it out?

I recently got a lot of hard built on junk off of one of my chickens using a drop of blue dawn dish soap in warm water and a qtip. It took several applications and also a toothpick once it started getting soft. I think they were as glad as I was to get it off!
For prevention, I just do a nose cleaning periodically now!
