Found 2.5 year old olive egger deceased in nest box

Last night we found out 2.5 year old olive egger deceased in the nest box with what appeared to be a complete prolapse of her vent. She had been dealing with what I thought was vent gleet for a few weeks, and hasn't laid in a few months. I could feel no eggs in her. She was eating again and acting normally except for the last few days she would hide when I would bring fresh veggies out and not try and eat with the flock. She was the head of the flock prior to this. The other girls werent bullying her. Any ideas on what could have been going on? I have one other girls who had a partial prolapse thati got back in and she still hasn't laid either but seems to be ok. I don't wanna lose another girl. They are my first flock. We have 11 girls left. All on Kalmbach layer 17% protein, free fed oyster shells, and fresh fruit and veggies a few times a week and meal worms for extra protein. Any advice would be appreciated.
 
Very sorry for your loss. Did she have any sign that she was pecked or had bleeding, or also had an egg present with her prolapse? Prolapses happen sometimes with straining to lay a lash egg, a stuck regular egg, straining to poop, and a dose of calcium can help increase muscle contraction to help lay. Are you feeding a layer type feed as the main food source, and do you have crushed egg shell available in a separate container from her feed?

There was bleeding but no signs of pecking. No eggs, and no sign of a lash egg. I feed them Kalmbach 17% protein layer feed as well as a separate dish of oyster shells and they get egg shell with hard boiled or scrambled egg a few times a week. The other hen who had the partial prolapse and is also still not laying in had given her calcium for a week when she was actively straining but I stopped because I thought you shouldn't give it to them longer than a week?
 
Yes, I would have stopped after a week of calcium as well. The bleeding could have been a sign that the others had pecked at her prolapse. Unfortunately, these things and other reproductive issues can be common in hens.
 
Your user name has me asking this: do you keep ducks with your chickens? And if you do, is there a drake among them?

The user name is because I had a rescue duck first. She was all by her self. Now its just the 11 hens and they aren't free range because of abundance of Hawks and other predators.

O yes, I just found your other thread when she was struggling the first time and I asked the same duck question.
