A five pound hen would get ~1/2 teaspoon twice a day.

For anyone that decides to use the Medistatin nystatin, when using the measuring spoon that it comes with, one level spoon = 3 grams.

Each gram = 400,000 IU, so one level spoon = 12,000,000 IU. The dose I use is 300,000 IU per kilogram, so one level spoon will treat 8-kilograms (17.6 pounds).

I found this in a few places for this same medication...

Mix 1 g per 20 ml of water and dissolve it well. Give 1 ml of this solution per 100g bodyweight for two times a day.

I think Plumb's Veterinary Drug Handbook says 200,000 to 300,000 IU per kilogram, so that amount is within the range.
