I would get some human vitamins from Walmart—vitamin E400 IU, human super B complex, and only give him a balanced chicken feed and a tablespoonful of scrambled egg daily. He could have wry neck from head injury, vitamin deficiency, or Mareks disease, hard to know which. A chicken sling or chair can be made to get him up and in front of food and water. If you lose him, keep his body chilled, but don’t freeze, and your state vet lab can do a necropsy and rule out Mareks. Here is a good thread with examples of chicken slings:
