Small Hole and deformed egg shell

Hello,
I am new to keeping chickens and one of my little ladies just laid a deformed egg. The tip looks wrinkled and there is a small hole. Is this something that just randomly happens or is it some sort of calcium/ nutrition issue? The Barred Rock that laid it has also recently had an issue with pulling out a chunk of feathers. She does not have any skin irritation or mites. I thought it was due to the crazy heat wave we had so i bought a fan and I started giving them frozen veggies as well as fresh.
 
Make sure to provide oyster shells for strong egg shells. That looks like either a toenail hole or a peck hole. Hot weather and stress can cause brittle shells. What are you feeding them as far as ration? I prefer something with a high protein than layer, sometimes hens get deficient on it.
