Not sure what’s wrong w/ 5mo chicken

The last couple days I have one chicken who’s been isolating herself from the flock and acting more lethargic. Usually she runs around with her sisters but she has been not been doing this as much. She sits by herself in the run and isn’t as active as normal (choose of to lay down while her sisters run around. When I go to let the chicks out to free range she has chosen to stay in the run the last couple times.

Today I picked her up and put her out side the run and she did move slowly to catch up with her sisters. When she tried to jump a small, 1.5 feet high, rock wall she’s done plenty of times before to get to where the others were she couldn’t clear it and she fell to the ground. It seemed like she was too weak to clear it like she normally does. Picked her up and put her on the other side of the wall and she caught up to the others. She stayed with them for a short while but now she’s completely separated herself again and is just lying by herself.

I noticed her poop today was green and white and on the watery side. And she has wet poop by the feathers by her vet.

Typically she runs away if I come near her but she’s not moving from where she is each time I approach her.

Not sure what is going on with her but she seems off.

Any thoughts?

Here is her poop and now she’s really hiding in an baby coop out in the yard that we no longer use. I’ve officially separated her. Not sure what I should treat for though? Dehydration? Coccidiosis?
