Very Sleepy and Un-energetic Chicken

I have never really encountered any symptoms of diseases or sickness from my chickens as I only have three and they are my first flock. Recently, my hen has been very drowsy and inactive. Her symptoms include yellow-ish diarrhea, lots of napping, and just not moving around very much. She's just been very quiet. She is around 5 years old. I would appreciate any help. I am very worried for her and I love her very much. Thank you!

I think the sleepiness has been going on for ~1.5 weeks. I don't think she has been really laying eggs. She isn't eating or drinking... I had to feed her some yogurt as from some research I figured it was a good way to give her probiotics. Her poop on the other hand was cleaned by my grandpa an hour ago (coincidentally). I will say it was yellowish and watery with white streaks in it. I didn't provide pictures as physically she looks completely fine. She is alert and does not appear to be struggling too much with moving. She is just very still.

Her comb is normal, still red but dry. She has been like this for ~1.5 weeks.
