Why does this chicken have a bald head?

This one chicken has a bald head, and it's been like that since we got her. Does anyone know what could be wrong or how we we fix it?

Hmmm...doesn't look raw or red like it's been picked at or a parasite like mites? CPB (chicken pattern baldness)?
