Any ideas on what might be wrong?

I can get photos tomorrow, Mondays and Tuesdays I leave before sunrise and get home after dark. She is eating and pooping ok. I feed them Purina Layena Pellets. She was hatched this spring and had been laying. My flock egg production is down a bit right now due to short days and I can not tell you for sure if she has been laying recently. I have done physical exams a few times since Friday and have not noticed any bloating. She will walk around and occasionally scratch, but spends a fair amount of time puffed up w/ her her tail down and neck tucked down. Her demeanor has improved slightly since I started wrapping it (maybe providing some pain relief), but no significant change in the appearance of her foot. I will definitely get more pictures ASAP.

If you Google Filiform Wart (in humans) that is exactly what it looks like. I can't find any articles about this virus in chickens, but it is identical in appearance.

Here are the pics I could get... since it's dark out. I get outside pictures tomorrow. The scabs on her toe and foot pad were where we removed a few thick scales on Friday, thinking we may get some drainage (if it was infected). Pink tissue was all that was found.

What do you feed, does she lay eggs, how long have you had her?
Crop emptying overnight?
Any bloat/swelling or fluid in the abdomen below the vent between her legs?

Check her crop first thing in the morning before she eats/drinks - it should be empty/flat.
If it's not empty, then I would begin treating for a crop issue.

The symptoms you describe sounds like she has another issue besides the foot problem.

The foot issue looks like Pododermatitis. Treat it like non surgical Bumblefoot with soaking. Let dry and apply a small amount of triple antibiotic ointment. Wrap the foot and keep her on dry bedding.

I can tell by the pictures she doesn't feel good. Beside what Wyorp Rock said, the only thing that she could benefit of change of is her roosting stick. It is a 2x4 which is good but, she needs to be standing on the 4" inch section as she's currently standing on the 2" section as this can be painful. Chickens stand on the entire bottom of their feet and don't use the perch like a parrot or parakeet. Make sure the sharpened edges are sanded smooth, not rounded, just smooth.

I would start with the coconut oil a couple of times and massage for the day, see if that helps clear it overnight.
Crop issues can take time to resolve so don't give up or expect to see complete results in a day or two.

Do keep in mind that crop issues are often a symptom of an underlying condition - reproductive disorders, worms, coccidiosis and infection are a few.
