Just found this in one of the nesting boxes. All my hens are fine, no prolapses. It's firm, with a density that reminds me of raw liver and there wasn't any blood. An

It is pretty bloody, and possibly a ruptured ovarian follicle. If it were more tubular it would probably be cecal core from coccidiosis. Others may have opinions. I will tag @Wyorp Rock to take a look. Are any hens acting lethargic, not eating, separating themselves, or have you seen any abnormal of soft eggs?
 
No, they all seem to be fine. No sneezing, running eyes, changes in their poop. I don't have a rooster though I have 3 male guineas. They spend the day looking for bugs and go in the coop before dark. The eggs count has been 0 for the last few days but I thought it was because something was getting the eggs. I put a camera in there today. The girls are mixed ages, 1-6 years old.
 
I also can't tell which hen it came from. As you can see, it isn't bloody. I didn't wipe it off, just picked off the shavings. I haven't given them anything new. Besides layer pellets, they get mealy worms or millet in the evenings.

I agree, it looks like an intact Follicle, maybe a couple of them, hard to tell.
